An innovative program of remote home care remedizer for patients with LVAD - implanted mechanical heart support single centre experience


Remedizer is a universal system of telecare (telemonitoring) of chronically ill patients together with an extensive pharmacotherapy module that allows for ongoing observation of one's own and their relatives' state of health as well as communication with doctors and coordinators.
At present, the system is applied in cardiology to look after patients with mechanical cardiac support (LVAD) while at home and under the care of the Instytut Kardiologii im. Prymasa Tysiąclecia Stefana Kardynała Wyszyńskiego w Warszawie, Szpital Specjalistyczny im. Jana Pawła II w Krakowie (Polska) and the ISMETT clinic (Italy). Currently, work is underway to implement care for patients who are candidates for heart transplants and post-transplant patients. It can also be successfully used for other groups of chronically ill people (e.g. diabetes).
The system is available in the form of a website and application for mobile phones (Android and iOS), which allows cooperation with external measuring devices.
PATIENT'S LOG – possibility of self-observation by patients. The application interacts with external measuring devices allowing for automatic transmission of results data via Bluetooth (e.g. blood pressure monitor, pulse oximeter, weight, temperature, pedometer, INR tester, glucometer) or manual entry (e.g. in case of communication failure – e.g. with LVAD pumps).
FIRST AID KIT / MEDICATION – contains information about medicines in the possession of the patient, their dosage and intake hours. Automatic reminders support therapy while reminders about medications that are about to run out eliminate the problem of their lack on the part of the patient (integration with the electronic prescription system is planned in the future). Thanks to the integration with the drug database, detailed information on their use, interactions alongside application alerts (e.g. contraindications for driving, alcohol consumption, breastfeeding or pregnancy) are available, which supports patients in their daily functioning.
COMMUNICATION –the system allows automatic transmission of health information to the monitoring unit. People with appropriate authorization receive alerts for data that goes beyond specific standards, which allows for faster response in case of complications and thus better patient care and lower costs. The system reminds about medical appointments, suggests vaccinations and supports communication between the doctor/treatment coordinator and the patient. In addition, the application allows the patient to transfer information about their health status to their loved ones.
The experience of using the system confirms the improvement of patients' quality of life and stabilization of their health.
The solution was awarded the Złoty Skalpel prize.
